  • Registration Form 
    A Registration Form should be used when registering for the next semester.  This form should be filled out and taken to the advisor.

  • Transcript Request Form
    Use this form to request a transcript sent from CSW to be issued to another party or to the student.  You may send the form and payment through the mail or you may fax us the form and pay over the phone with Credit Card.

  • Request a Transcript From Another School 
    Use this is a form to request a transcript from a previously attended school to be sent to CSW.

  • Concurrent EnrollmentA student planning to enroll concurrently at CSW and another college or university must complete a Concurrent Enrollment form, obtain approval signature from his or her academic advisor, and file the form with the Office of the Registrar in advance of each semester during which the student plans to be concurrently enrolled.

  • Status Change
    If you have changed your address or your name then we need this form from you.

  • Challenge By Exam

    CSW students wishing to challenge a course offered at CSW must complete this form and follow the appropriate steps for approval to challenge the course. Advanced Standing examinations may be taken in most areas. Students who make a grade of "B" or higher on the end-of-course examinations are administered by the appropriate college department may receive credit for the course. Examinations are available at a cost to students of $50.00 per course and a recording fee of $25.00 per credit hour. Credit by Challenge Examination is limited to 6 hours per student. Contact the Office of the Registrar for more information. 


  • Credit By Exam                                                                              Entering freshmen whose ACT/SAT scores place them far above the national average can be awarded up to 27 hours of college credit at CSW in the following areas: English, Math, Natural Science, and Social Science. The college will grant credit for acceptable scores on the exams, credit for general exams for which a student earns a score of 50 or higher are in the areas of: Composition & Literature, Science and Math, Foreign Languages, and History and Social Sciences. The college will grant advanced placement credit for courses on the basis of acceptable scores on the Advanced Placement Examination of the College Entrance Examination Board. There will be a $25.00 fee per credit hour to record the credit on the official transcript. Submit the application and official test scores to be evaluated by the Registrar.and return it to the appropiate dean.  ***Please see current catalog for more details.


  • Credit By Experience 
    CSW students wishing to apply for credit based upon experience must complete this form and return it to the appropiate dean.
  • Drop, Add, and Withdrawal Form
    If you are wanting to drop or add a class this would be the form for you to use.  Also if you would like to withdrawal from a class this form would be used.  We need this form signed by you before we will make any changes.
